Whether you have an additional vision policy or it's covered under your standard policy, make sure you have the card available. You'll need the information on the card for us to process it.

Have Physician's Information Ready

Bring the name and contact information of your primary care physician. You'll need to fill this information out on your medical history report.

Comprise a List of Any Medications You Take

Your medical history report will also ask you about the medications you take and their dosages. It helps to write this information down on a piece of paper or insert it into a memo on your phone, so you have it available when you come in for your appointment.

Ask a Friend or Family Member to Be Your Driver

Your yearly exam will include a pupil dilation exam. Many people can't see well enough to drive safely after this test. Therefore, you should have someone to drive you home just in case you can't see after this part of the test.
